Streamlining a general surgeon’s clinical correspondence: The Medow and Gentu workflow

One general surgery workflow, seen from both sides

Dr Siva, is a general surgeon working across public and private practice. He consults and operates at Lakeview Private Hospital and Dubbo Private Hospital, and holds public appointments at Dubbo Base Hospital and Mudgee Base Hospital.

His work spans a broad range of general surgery, with particular interests in advanced laparoscopic procedures, hernia surgery, skin cancer surgery and endoscopy. He also performs breast, thyroid and gastrointestinal surgery, alongside a wide range of general surgical procedures.

That variety carries through to his correspondence.

A patient may first be referred by their GP, attend an initial consultation, complete further investigations, move into surgical planning and then return for post-operative review and follow-up. Each stage creates a different communication need.

A consultation letter may need to bring together the patient's history, examination findings, investigation results and recommended plan. Surgical correspondence needs to clearly explain what has been agreed and what happens next. Post-operative and follow-up letters need to keep the GP informed as the patient moves through recovery.

Dr Siva creates and reviews that clinical content. Pene, his practice manager, works on the other side of the same workflow, making sure every letter reaches the right recipient at the right stage of the patient journey.

Medow changed that experience for both of them, but in different ways.

For Dr Siva: correspondence built around general surgery

Dr Siva mainly uses Medow for GP letters and consultation notes. Across a practice as varied as his, those letters can look very different from one patient to the next.

A hernia consultation does not follow the same structure as an endoscopy finding, a skin cancer review or a post-operative update. The terminology, level of detail and next steps change depending on the procedure, the patient and the stage of care.

Medow's specialty-specific model for general surgery gives Dr Siva a starting point designed around the language and documentation patterns of the specialty. From there, the output can be refined to better reflect his preferences.

The letters are becoming much closer to the way I want them structured and written.

Over time, the output has become more aligned with his preferred phrasing, structure and level of detail. GP letters and consultation notes now require relatively little correction before they are ready.

The clinical judgement, however, remains entirely his. Medow captures and structures what Dr Siva says; it does not introduce its own interpretation of the consultation or make clinical decisions on his behalf.

The AI documents what I say. It doesn't put its own thoughts into the letter or introduce its own diagnosis. The judgement stays with me.

For Dr Siva, that distinction matters. Medow supports the documentation, while the final assessment, wording and approval remain with the surgeon.

A shorter route from dictation to review

Dr Siva had always dictated his letters. Before Medow, those recordings went to an experienced typist, who transcribed them into Gentu. Dr Siva then reviewed the finished letter before Pene could send it.

The typist understood his terminology and knew how he liked his correspondence structured. The quality was not the problem. The limitation was the extra handover and the reliance on one person being available.

If the typist was unwell, on leave or eventually retired, the practice needed another way to keep correspondence moving. Even when everything ran smoothly, Dr Siva still had to wait for the transcription to return before he could review it.

Medow removes that delay. Dr Siva dictates, reviews the generated letter and approves it without waiting for a separate transcription stage.

Nowadays, because of Medow, I can review the letter, make any small changes and get it back to the GP much faster.

For Pene: correspondence ready when she needs it

Pene sees the same workflow from the practice management side.

Her role is to make sure correspondence continues moving as the patient progresses from consultation to surgery, post-operative review and follow-up. Before Medow, she could only take the next step once the dictation had been transcribed, reviewed and returned to her.

Now, once Dr Siva approves a letter, it appears directly in Gentu.

Everything is much more immediate now. The letters are there when I need them, and I haven't had to chase anything up.

That changes the day-to-day experience of managing the practice. Instead of checking where a letter is in a separate transcription process, Pene can focus on reviewing the outgoing correspondence, sending it and keeping each stage of communication in the right order.

The Gentu integration is central to that handover.

The integration with Gentu is almost instant, so the turnaround is far quicker.

For Pene, this means correspondence arrives in the system she already uses, ready for the next step. There is less waiting between the clinical and administrative sides of the practice, and greater certainty around when letters will be available.

Support from someone who knows the practice

For Dr Siva and Pene, that person is Isabel. She knows their practice, understands how they use Medow with Gentu and is familiar with the way Dr Siva wants his correspondence structured.
